NEW PATIENTS We understand that you are probably reaching out to us during a difficult or challenging time, and we want to make the use of our counseling services as easy as possible. Please contact one of our individual therapists directly via email, phone/voicemail, or text. You may also contact David or Donna Rennard to discuss your needs.
Your therapist will contact you directly to talk with you and determine if your counseling needs can be met competently and effectively by him or her. If you agree to a subsequent meeting, your therapist will schedule a session and forward you the appropriate intake forms to be completed. A link to these forms can also be found on the Client Information page.
All our therapists offer in-person and telehealth counseling sessions. However, it is considered “best practice” and recommended that the first session be in-person to provide the best assessment of your needs and determine if you and your therapist are a “good fit.” Participation in counseling is always voluntary and we want to make sure that you are confident in your choice of a therapist.
We highly respect our clients’ health concerns and do our best to accommodate them. Our facility is fully accessible to all persons. We also continue to maintain social distancing in our offices and will gladly comply with our clients’ requests regarding proper masking during this pandemic.
INSURANCE COVERAGE We will bill your insurance company for the services provided if you sign the consent form in the intake packet and provide us with all necessary information. We want you to be fully informed about the use of your insurance and experience a smooth process. If you have questions or concerns, please contact your therapist. Please note the following:
It is highly recommended that you contact your insurer to verify coverage for the services you receive from Aspen Counseling Services and your individual provider, check to see what your current deductible amount is, and see if you are liable for any co-insurance or co-payment amounts.
Your therapist may be empaneled with your insurer and be considered “in-network.” In that case, our charges for your services are set by our contract with your insurer. If your therapist is not empaneled with your insurer, our standard rate for services will be charged. You can find a listing of those fees through the “No Surprises” Act link on the Client Information page.
You are fully responsible for all payments due, not your insurance company. Even if Aspen Counseling Services is an empaneled provider for your insurance company, your particular contract with your insurer governs all matters. Aspen Counseling Services will act in good faith to rectify any errors in billing for your account, and we will resubmit billing if requested. Any additional negotiation with your insurance company and its contractual obligations will be your responsibility.
INTAKE FORMS Our standard Intake Packets for individual and group counseling are available below for you to review and complete. If you have already registered for individual counseling, you do not have to complete a group packet.
A Release of Information form allowing contact with other providers or family members, as well as a supplemental Telehealth Agreement are also available.

CREDIT CARD AGREEMENT All our clients are asked to provide a credit/debit/HSA card authorization form for convenience of billing. This is a voluntary request. A standard bi-monthly billing by email or U.S. mail can be arranged. A statement of account can be requested at any time.
“NO SURPRISES” ACT Aspen Counseling Services is required to provide a good faith estimate of the cost of all services provided. If you have any questions regarding our fees or how you have been charged for services, please contact your therapist.
